At Main Street Dental our focus is on you. Your new patient appointment is intended for you to get acquainted with Dr. Messinger and the people at our office as well. We are a family owned and operated practice and a vibrant part of the community, not a franchise. There won't be a doctor at our practice whom you've not met. Your new patient appointment will consist of digital X-rays (panoramic and bite wings) and a comprehensive oral evaluation by Dr. Messinger. We will also perform an oral cancer screening and perio probing of your gums at no charge. When you first arrive at our office you will be asked to review, complete, and sign the new patient paperwork. To pre-register, we have provided these new patient forms for you to print prior to your first visit.

This paperwork will consist of the following:

  • Medical History
  • Main Street Dental’s Practice Policies
  • Health Insurance Portability Accountability Act
  • Parental Consent to Treatment (For Minors Only)

We do recommend a dental cleaning at your first new patient appointment. However, we do understand that you may be in pain or your schedule doesn’t allow for cleaning at the initial appointment. We recommend you discuss this with our appointment coordinator when you call to make your first initial appointment with us.
